Where our services involve access to, or processing of, Protected Health Information (PHI) on behalf of a client, that relationship is governed by a separately executed Business Associate Agreement (BAA) consistent with the Health Insurance Portability and Accountability Act (HIPAA) and, where applicable, Texas House Bill 300. This website’s contact form is not intended for the submission of PHI, and you should not include patient or resident health information in any form field, text message, or voicemail directed to us through this site.
Your Rights & Choices
Depending on your location, you may have the right to request access to, correction of, or deletion of your personal information, and to opt out of marketing communications at any time.
